Government forms are legal documents produced by the departments and agencies of the U.S. government, including the Internal Revenue Service (IRS), U.S. Citizenship and Immigration Service (USCIS), Social Security Administration (SSA), Department of Motor Vehicle (DMV), Department of Army (DA), and Department of State (DA).

Form W-9, Request for Taxpayer Identification Number and Certification

Companies that hire self-employed workers use Form W-9 to report or file their tax to the Internal Revenue Service (IRS). If you are a self-employed professional, freelancer, independent contractor, or consultant in the United States, you need to furnish your Form W-9, Taxpayer Identification Number (TIN), Social Security Number (SSN), Individual Taxpayer Identification Number (ITIN), Adoption Taxpayer Identification Number (ATIN), or Employer Identification Number (EIN), to your employer so he or she can report the amount paid to you. You may also be required to file Form W-9 in certain real estate transactions, paying mortgage interest, paying student loan interest, cancellation of debt, and earning interest from a bank.

Form 1095-A, Health Insurance Marketplace Statement

You will receive Form 1095-A from the marketplace if you enrolled in a qualified health plan through the Health Insurance Marketplace. You need to fill it out to report your information and insurance coverage. Form 1095-A is also used to calculate the amount of your premium tax credit and reconcile advance payments of the premium tax credit made on your behalf with the premium tax credit you are claiming on your tax return. It is necessary to accomplish it before you file your tax return using Form 8962, Premium Tax Credit.

Form SS-5, Application for a Social Security Card

Form SS-5 by the Social Security Administration is a form used to apply for an original and replacement Social Security card and change or correct information on Social Security Number record.

Form 1040, U.S. Individual Income Tax Return

Use Form 1040 to file your income tax returns. It is one of the three personal income tax forms issued by the IRS. However, there are certain qualifications to know whether you are eligible to file Form 1040. It includes your marital status, age when filing, and gross income. Some credits require you to file Form 1040, such as earned income, additional child tax, American opportunity, credit or federal tax on fuels, premium tax, and health coverage tax. These credits will require you to attach documents, including but not limited to Form 1040A U.S. Individual Income Tax Return, Form 1040 Schedule 8812, Additional Child Tax Credit, and Form 1040 Schedule R, Credit for the Elderly or the Disabled. If you made a mistake on your already filed Form 1040, use Form 1040x, Amended U.S. Individual Income Tax Return, to correct it. There are more refined and suggested versions of Form 1040, such as Form 1040 and Form 1040EZ, Income Tax Return for Single and Joint Filers With No Dependents. Use Form 1040A if your taxable or combined income is below $100,000, you have capital gain distributions but no other capital gains or losses, and you do not itemize deductions. Whether you are filing single or jointly, as long as you and your spouse are younger than 65, were not legally blind during the last tax year, have no dependents, your interest income is less than $1,500, and your income or combined incomes for joint filers is less than $100,000, you can file Form 1040EZ.

Driver License Application forms

Use a Driver License Application form to register for a driver's license, learner's permit, state identification card, hearing impaired ID card, or an emancipated minor ID card in your state. If you need to replace your driver license record, use a Driver License Replacement form. If you need to renew your driver license, use a Driver License Renewal form.

Form DS-230, Application for Immigrant Visa and Alien Registration

If you are an alien and want to migrate to the U.S. permanently, you must file Form DS-230 with the U.S. consulate to obtain an immigrant visa. If you prefer to file electronically to obtain an immigrant visa, you can file Form DS-260, Immigrant Visa Electronic Application.

Form AR-11, Alien's Change of Address Card

Non-U.S. citizens who intend to stay in the United States for more than 30 days use Form AR-11 by the U.S. Citizenship and Immigration Service (USCIS) to report a change of address within ten days. Failure to update the address may charge them with imprisonment or removal or a $200 fine. They cannot obtain future visas or receive immigration benefits.

Form DA 348, Equipment Operator's Qualification Record (Except Aircraft)

Army service members use Form DA 348 by the Department of the Army (DA) to determine their equipment operating qualifications and driving experience. It is a form used in the U.S. to ensure that every service members are qualified to operate specific types of army equipment. It contains information about the service member's official qualifications, background and experience, performance record, driving performance test, and road test. Attach Form DA 348-1, Equipment Operator's Qualification Record (Except Aircraft), to Form DA 348 if there is an insufficient space for results.

Form DA 5960, Authorization to Start, Stop, or Change Basic Allowance for Quarters (BAQ), and/or Variable Housing Allowance (VHA)

Use Form DA 5960 to start, adjust, or terminate a military member's entitlement to basic allowance for quarters and or variable housing allowance.

Form SSA-3368-BK, Disability Report Adult

Use Form SSA-3368-BK to apply for Social Security Disability (SSDI) or claim disability benefits for adults. It must be filed with the Social Security Administration. You are eligible to file Form SSA-3368-BK if you are disabled. The Social Security Act defines disability as an individual's inability to work that is expected to last or has lasted for at least a year or is expected to result in death. Evidence of disability is required. Thus, you need to provide certification from a medical doctor and the contact information of a representative to vouch for your claim.
