Finally, for this section, you’ll need to choose your Race. Place the following codes for the corresponding ethnicity.
(A) if you are either Asian or a Pacific Islander
(B) if you are Black
(H) if you are Hispanic or
(W) if you are White or Caucasian
(I) if you are an American Indian / Alaska Native
(O) if you are Other, or none of the above.

For this section, you’ll have to indicate your contact information or how you’ll be contacted.
You’ll need your Home phone number, other phone number, and email address.
Address Information
You’ll need to input your residence address. This is where you currently reside in. You must also indicate your city, state, county, and ZIP code.

Information Form
This part of the form will require you to tick the “Yes or No” boxes for the statements that apply or do not apply to you. This part of the form has two sections, one for general information and one for medical information.
General Information
You will first be asked to indicate whether or not you are a U.S. citizen. Click Yes or No depending on whether you are one or not.

If you answered yes to the previous question, you’ll be asked if you are a 60% disabled Veteran receiving compensation and want to waive the application fees.
At the end of this section, you’ll be asked if you want to provide two emergency contacts to be reached in the event of injury or death. If yes, you’ll need to indicate their names, telephone numbers, and addresses.
Medical Information
The first question of this part is whether you have been previously diagnosed with any medical condition that may affect your ability to safely operate a vehicle. This includes, but is not limited to:
If you answered “yes” to the question above, you will be asked to indicate whether your condition has either improved or deteriorated since your last application of a license.

Well done, you’re almost through with the application form! To finish up, you just have to acknowledge a few things.
If you are an able-bodied male citizen or immigrant, who is at least 18 years of age but lower than 26, submitting this form consents to registration with the United States Selective Service System.
Being registered means that you qualify for federal student aid, job training, federal employment, and citizenship if an immigrant. In Texas, you must register to qualify for federal student aid or state employment.
If convicted, failure to register is considered a felony punishable by up to five (5) years in prison and/or a fine of $250,000. If not registered by age 26, you could permanently lose out on these benefits.
Before finishing, you’ll be asked to certify all this information by indicating your residence whether its a single family dwelling, apartment, motel, or temporary shelter.
To certify all the information that has just been provided, you will be asked to sign the document and indicate the date of accomplishment.
